
The AUTO-FORM brand was established in 1974. Initially, the company's business was tinsmithing repairs.

At the turn of the 1990s, the owner of the company, Mr. Jerzy Kuc, developed a concept of converting Polonez passenger cars into ambulances. Together with an employee, Mr. Roman Pietrzyk - a multiple Polish champion in flying models, they created their own structure of extending and raising the Polonez car to meet the needs of an ambulance. It was a breakthrough moment in the history of the company, the basis of which became the production of ambulances.

Since 1995, apart from the Polonez car, the company has also started to produce ambulances based on cars of other brands. Initially it was Toyota Hiace, then Peugeot Boxer, Renault Master and Mercedes-Benz. Currently, the company performs bodywork mainly on Mercedes-Benz, Renault, Volkswagen and Opel cars. It also has certificates confirming membership in the VANPARTNER and RENAULT SAS partner programs.


After years of operation in Mysłowice, in 2018 AUTO-FORM moved the company's headquarters to Sosnowiec, to a modern building at ul. Inwestycyjna 5. In the same year, a dedicated Ambulance service system was implemented, which was financed thanks to EU funds under the Regional Operational Program of the Śląskie Voivodeship.
